Web Order Form
Please use this form to place an order for a Teaching Evaluation (TEVAL)
TEVALs need to be ordered at least 6 working days before they are due to be conducted.

Orders are only accepted from the lecturer or tutor being evaluated . A separate TEVAL needs to be ordered for each lecturer or tutor being evaluated.

You need to place a separate order for each class group . For example, if you tutor three groups in the same course you need to place three separate orders.
Please read the points below carefully, if they are not followed we will NOT process your order.

 You must zero fill single digits i.e. 01.
Do not uses slashes i.e. / or -.
Use only 2 digits for the year i.e. 06.
Time you will conduct the evaluation e.g. 1500
 Use 24 hour format with no spaces, letters, or slashes
If class is externally delivered write "External"
